Welcome to SurveyTel, a digital survey platform where we build a community to allow members the ability to have their voice and opinions heard by major stakeholders. The goal is to improve the various telecom/digital product and services that are used by our members through the opinions provided per each survey. Registration is completely free and are open to individuals.

Please read these Terms and Conditions carefully before using the digital survey platform of SurveyTel (“platform”, “portal”, “website” or “survey”) operated by SurveyTel ("us", "we", or "our"). Your access to and use of our Portal is conditioned on your acceptance of and compliance with these Terms. These Terms apply to individuals who access or use the Portal for any Service

1. By clicking “I agree”, or by submitting your information on any SurveyTel platform, you agree to the terms and conditions and the Privacy Policy described herein (“collectively referred to as “Terms”). Please note that this Agreement or any terms of it is subject to change, modify and update at any time at the sole discretion of SurveyTel without prior notice (unless prior notice is required by law) and the last updated version of the terms shall apply. By completing this online questionnaire, you agree to be legally bound by these terms, which shall take effect immediately on your use of this website. If you do not agree to be legally bound by all the terms, please do not access and/or use the website and/or complete the questionnaire.


2. You agree that SurveyTel may collect, store and process personal information (including special personal information) for purposes of (i) providing the services, (ii) maintaining its internal administrative or client relationship management systems, including the use of third party IT outsourced providers, (iii) quality and risk management reviews and legal reporting obligations, (iv) providing information about its range of services, (v) carrying out and managing its business operations,  (vi) any legitimate business purposes of or other activities conducted by SurveyTel from time to time.  You undertake to keep the personal information up to date by keeping SurveyTel informed of any changes that need to be made to the personal information.


3. You further agree and undertake to immediately inform SurveyTel (i) of any changes to the personal information provided by you in order to keep the data accurate, (ii) should any data subject withdraw any consent previously given and/or (iii) should SurveyTel for any other reason no longer be entitled to lawfully process the data subject’s personal information.


4. You agree that SurveyTel shall be entitled to retain the personal information for a minimum period of 5 years, after which, subject to what is set out below, the personal information shall be destroyed in accordance with the provisions of SurveyTel.  You agree that subject to appropriate safeguards, SurveyTel shall however be entitled to extend the period for which personal information is retained if (i) this is required or authorised by law, (ii) SurveyTel reasonably requires the information for a longer period for lawful purposes related to its activities or functions, and/or (iii) for statistical or research purposes.


5. SurveyTel is not liable for any damages (including direct, indirect, consequential, incidental and exemplary damages) in the event that this site is unavailable to users (by virtue of interruption, suspension or termination) for any reason, including due to computer or communications link downtime attributable to malfunction, upgrades or preventative or remedial maintenance activities.


6. SurveyTel will not be liable to users in respect of any loss or damage (including direct, indirect, incidental, consequential or inconsequential loss) which may be suffered or incurred by users or which may arise directly or indirectly because of services supplied by SurveyTel.


7. SurveyTel will not be liable for any loss caused by the failure to complete the questionnaire.


8. Users agrees that all information provided by them on the questionnaire is accurate and does not breach any law or the rights of any person.


9. You may not sell or modify the material or reproduce, display, distribute, or otherwise use the material available on this website in any way for any public or commercial purpose. You may not copy or adapt the code or software that SurveyTel creates to generate its pages.


10. You may not ‘frame’ or ‘mirror’ any materials or third party content contained on or accessible from this site on any other server or internet based device without the express written authorization of SurveyTel.


11. SurveyTel will not be liable for any damages (including direct, indirect, consequential, incidental and exemplary) if there is any deficiency or inaccuracy in the site attributable to a lack of maintenance of the site or in relation to the accuracy, sufficiency or otherwise of your application.


12. Upon any breach of this agreement, or inappropriate use of SurveyTel services as determined by SurveyTel in its sole discretion, SurveyTel reserves the right to terminate your registration. SurveyTel’s preferred course of action is to advise you of your inappropriate use or breach of this agreement and recommend any necessary corrective action.


13. We are committed to protecting your personal information and respecting your privacy. Personal information is defined as any details that will enable you to be identified, such as ID numbers, telephone numbers, address, email address etc.


14. When designing and executing our survey, it is our policy to take all necessary steps to ensure that personal information you provide is processed fairly and lawfully. It is our policy to monitor our internal procedures regularly to ensure compliance with the relevant statutory requirements in all that we do.


15. Only authorised staff has access to personal information and they are obliged to respect its confidentiality. We do not sell, rent or exchange any personal information supplied by you to any third party. Nor do we use any of the information you provide for direct marketing or other non-survey study activities.


16. In obtaining your cooperation to participate in the survey, we undertake not to mislead you in any way about the nature of the survey we are conducting, the way in which the data is collected and the use that will be made of the survey results.


17. All of the information that you provide will be treated as confidential and will only be used for study analysis purposes. Your comments will not be identified as belonging to you, instead they will be combined with those gathered from other survey participants, and will be analysed as part of a group.


18. If we ask you for personal information that enables you to be identified - e.g. your name, ID numbers, e-mail address or telephone number, we will clearly state why we are asking for it and for your permission to use it for that purpose. For example, it might be to contact or invite you for surveys or for follow-up research.


19. Your participation is voluntary. You are entitled to ask that part, or all, of the record of your involvement in the survey be deleted or destroyed.
